The Old Spaghetti Factory menu at 53 Water St, Gastown, Vancouver:

Every entrée includes hot sourdough bread, your choice of Minestrone Soup or Crisp Green Salad, Spumoni Ice Cream, and coffee or tea. Dressings: Ranch, 1000 Island, Blue Cheese, House Italian or Low-Fat Basil Balsamic. Substitutions (Caesar, Spinach Salad, Clam Chowder, French Onion Soup) are available for a small upcharge.
